In modern dentistry, soft tissue reconstruction around teeth and implants is developing to meet increasingly complex challenges. Constantly increasing patient demands require more and more advanced concepts of soft tissue management. Establishing a healthy situation based on the achievement of healing by primary intention without scar formation is still the main goal of any surgical treatment in the oral cavity. While functional aspects are clearly the focus in the posterior area, a harmonic natural contour of the gingiva is paramount in the esthetic zone. Soft tissue corrections are therefore often inevitable to compensate for previous volumetric losses and to ensure accessibility for good patient oral hygiene. A precise planning of the initial incision, the preparation of surgical flaps and the final suturing technique are essential for a successful treatment outcome. Understanding the histologic composition of the palatal mucosa as the predetermined harvesting bed as well as understanding the biologic interactions around teeth and implants in the course of soft tissue autografting is in this context fundamental for contemporary soft tissue management and for meeting the high demands of patients. Important procedures from the surgical step-by-step methodology to the design of the final restoration will be discussed to offer practical support to dentists in their daily clinical practice.
