Dr. Renato Cocconi is an orthodontist in Parma, Italy where he works at a private clinic that he directs with Dr Raffaini. Dr Cocconi attended a 3 year Postgraduate Program in Orthodontics at the Department of Orthodontics of the University of Milan where he obtained his master’s degree in 1993. He subsequently spent 2.5 years in the United States where he worked in L.F. Andrews Foundation and attended the Roth Williams postgraduate course in from 1991-1993. Since that time Dr Cocconi has been very active in the orthodontic community nationally and internationally. His accomplishments and collaborations worldwide include working with Dr Terry Tanaka at USC, election to the Scientific Committee of the Angle Society and countless invitations to speak internationally for such groups as European Orthodontic Society, AAO, the American Academy of Esthetic Dentistry, the European Academy of Esthetic Dentistry and the World Orthodontic Meeting in London in September 2015. He has served as a clinical professor in the Department of Orthodontics of the University of Milan from 1993 to 2008. Currently, Cocconi is active in many organizations including the EOS, AAO, SIDO and the Angle Society of Europe. He will be part of the European Team (with Mauro Fradeani and Mirco Raffaini) selected for the Meeting "USA meets Europe" that takes place in Tegernsee Germany every 4 years, organized by the Seattle Study Group, DGAZ and AIOP.

Bilateral or unilateral congenitally missing maxillary lateral incisors is a common clinical situation that requires an interdisciplinary approach. It is often complicated by the presence of narrower teeth.
Occlusal, periodontal, and esthetic considerations will influence the decision to close the space for a canine-lateral substitution or to open the space for a prosthodontic replacement of the missing lateral incisor. The team should define the proper dental position to provide a stable occlusion and dental esthetics obtained with a minimally invasive preparation. The restorative approach should provide a long-term functional and esthetic solution in the transitional period from adolescence to adulthood, reversible for future possible interventions.

The digital technology can be a GPS in designing a multidisciplinary treatment that involves orthodontics and restorative dentistry. A proper hierarchy of decisions and responsibilities need to be defined. Form is everything but position and size. For this reason the orthodontist first needs to set up the proper occlusion deciding the position and the available size that will be necessary for the restorative dentist to obtain the proper form with a minimally invasive preparation. This case report illustrates with a step-by-step approach all the clinical issues of the treatment, from diagnosis to orthodontic guided position of Tads, materials of choice and laminate veneers realization passing through all the interactions between the dental team members explaining who does what and when and proposing a clear hierarchy of decision. A review of digital and classical possibilities for the realization of laminate veneers is proposed with its pros and cons.
Several treatment options are described in the literature to solve the microdontia-related problems of maxillary anterior teeth. These options are often focused on an orthodontic rather than a restorative approach. The case presented here started with an orthodontic phase followed by a restorative one. The aim of the orthodontic phase was to achieve the correct spacing, respecting ideal average tooth proportions. This was done using a digital setup to restore the six anterior teeth, so that an optimal esthetic and functional result was achieved by the end of the orthodontic treatment. The purpose of the restorative phase was to contribute to the esthetic outcome and occlusal stability. The 12-year-old patient was treated utilizing a CAD/CAM procedure, which took advantage of the mechanical properties and reasonable cost of the chosen resin matrix ceramic material (Vita Enamic). Once stability has been attained and proper periodontal tissue maturation has been reached at the end of the patient's growth, there will then be the option of improving the esthetics by replacing these restorations with ceramic veneers.
Zur Lösung von Problemen aufgrund einer Mikrodontie im Frontzahnbereich sind in der Literatur diverse Therapieoptionen beschrieben. Meist beruhen sie schwerpunktmäßig auf einem kieferorthopädischen und weniger auf einem restaurativen Ansatz. Der hier vorgestellte Fall begann mit einer kieferorthopädischen Behandlung und endete mit einer restaurativen Phase. Kieferorthopädisches Ziel war eine korrekte Lückenverteilung im Hinblick auf ideale durchschnittliche Zahnproportionen. Dies wurde mithilfe eines digitalen Set-ups zur Wiederherstellung der sechs Frontzähne erreicht, um nach der kieferorthopädischen Behandlungsphase ein optimales ästhetisches und funktionelles Resultat zu erhalten. Die restaurative Behandlung diente dazu, die Ästhetik und die Okklusion zu verbessern. Die 12-jährige Patientin wurde mit CAD/CAM-gefertigten Restaurationen aus einer Hybridkeramik (Vita Enamic) versorgt, die sowohl gute mechanische Eigenschaften bieten als auch kostengünstig sind. Sobald die Patientin ausgewachsen ist, stabile Verhältnisse bestehen und die Weichgewebe vollständig ausgereift sind, kann die Ästhetik optimiert werden, indem diese Restaurationen gegen Keramikveneers ausgetauscht werden.
