Preface
Orthodontic-surgical treatment of dentofacial deformities is a continually evolving discipline whose practice is becoming more widespread. This book was written for orthodontists and maxillofacial surgeons who are already working in this fascinating field, as well as for those who are preparing to do so. Drawing on more than 30 years of clinical experience, I have illustrated in this text the essential aspects of treatment of dentofacial anomalies while avoiding long theoretical explanations. My primary aim is to achieve clarity and simplicity so that orthodontists can more easily understand the surgical problems involved in such treatment, and likewise surgeons can become more familiar with the orthodontic aspects.

The philosophy that must guide us in treating dentofacial deformities is that of integrating function and esthetics. Technologic progress gives us increasingly sophisticated instruments, but nevertheless, underlying a good result and a good relationship between doctor and patient must always be a large dose of clinical common sense. For this purpose, I have provided practical information related to the various therapeutic phases-from the initial planning through the orthodontic and surgical phases and finally to the postoperative reeducation. I have also given particular importance to some aspects that have recently emerged in the literature: relationships with temporomandibular joint dysfunction, problems surrounding rigid fixation and condylar position, aspects of postoperative rehabilitation, and the possibilities that may be offered by new surgical techniques such as distraction osteogenesis. There is also an important chapter concerning how to prevent and manage errors and complications. The final chapter is devoted to presenting and discussing 33 clinical cases that exemplify the different types of anomalies as well as different therapeutic solutions. The DVD accompanying the book illustrates a complete case from start to finish with particular emphasis on surgery. The entire procedure is presented, including footage of all the principal stages.

I hope that this work will be a simple and practical tool for study and consultation for anyone who treats patients with dentofacial anomalies, whether it is to review a single aspect in greater depth or to learn how to avoid some of the most frequent errors. Indeed, if this work helps only one colleague to better understand one aspect of treatment or avoid a mistake, then I can say that I have truly achieved my most important goal.
