Inflammation and Host Response in Oral Squamous Cell CarcinomaBackground: Role of inflammation in the tumour microenvironment has now been widely accepted, and many studies have linked as much as 20% of cancers to chronic infections. In the oral cavity, the junction of hard and soft tissues is very prone to infection and acts as an ecological niche to micro-organisms. Periodontal disease or periodontitis is a unique inflammation which is persistently present in the oral cavity and, hence, its association with oral cancer becomes mandatory to study. The disease progression depends on a complex interaction between the tumor and the host's inflammatory response. Hence, as an adjunct to malignant cells behaviour, predicting host response in terms of blood vessels and lymphocytes density is gaining ground these days.
Methods: Three groups of patients will be studied. First group, comprising individuals with a history of tobacco chewing of more than 3 years but without developing any lesion. Second group will contain individuals with a history of tobacco chewing manifesting with the development of precancerous conditions. And the third group, with the individuals who chew tobacco and have developed oral cancer. Oral hygiene status and the periodontal condition as local infection factors will be correlated with those of systemic inflammatory indicators, including C-reactive protein and ESR, among the three groups.
Hypothesis: The association between local and systemic factors will help us to understand whether it plays any significant role in the progression of oral squamous cell carcinoma among patients of the same age and sex.

Ziel: Ziel vorliegender Studie war zu untersuchen, ob eine einfache Selbsteinschätzung der individuellen Stressbelastung in der Zahnarztpraxis geeignet ist, um Patienten mit einem erhöhten stressbedingten Risiko für Parodontitis zu identifizieren und die Erhaltungstherapie darauf abzustimmen.
Methoden: Die Studienpopulation schloss 289 Patienten mit Parodontitis ein. Alle Probanden beantworteten einen Fragebogen, der im Rahmen des bevölkerungsbasierten Forschungsprojekts "Popgen" des "Nationalen Genom Research Network" und der Universitätsklinikum Schleswig-Holstein erhoben wurde, und u.a. vier Fragen zur Selbsteinschätzung der individuellen Stressbelastung beinhaltete. Außerdem wurden Fragen die gingivale Entzündungszeichen betreffend gestellt (Blutungen und Schwellungen). Patienten, bei denen an mindestens einem Drittel der vorhandenen Zähne eine approximale Knochenreduktion von ≥ 30 % der Wurzellänge radiografisch nachgewiesen werden konnte, wurden als Patienten mit generalisierter Parodontitis klassifiziert.
Eine Prüfung auf signifikante Häufigkeitsunterschiede zwischen den Gruppen erfolgte mittels Chi-Quadrat-Test. Die Berechnung des Korrelationskoeffizienten nach Spearman diente dem Nachweis eines möglichen Zusammenhangs zwischen Stress und Gingivitis oder Stress und parodontalem Knochenabbau.
Ergebnisse: Blutungen und Schwellungen der Gingiva wurden signifikant häufiger von Patienten berichtet, die oft unter Traurigkeit und Lethargie litten (Chi2 Test, p < 0,001). Die Antworten von Fragen zur Stressbewältigung und langfristigen starken Stressbelastung zeigten keine signifikante Assoziation mit gingivaler Entzündung (p > 0,05). Die Gesamtstressbelastung korrelierte mit verschiedenen gingivalen Entzündungszeichen. Andererseits konnte kein Zusammenhang zwischen dem Auftreten einer generalisierten Parodontitis und häufigerer oder stärkerer Stressbelastung nachgewiesen werden.
Schlussfolgerung: Einfache anamnestische Angaben zur Stressbelastung können den bekannten Zusammenhang zwischen Gingivitis und Stress widerspiegeln. Eine korrelation mit einem erhöhten Parodontitis risiko wird jedoch nicht belegt

Background: Cardiovascular disease accounts for an estimated 40% of all deaths in New Zealand, with atherosclerosis as the underlying aetiology in the vast majority of cases. Deaths are higher among Maori compared with non-Maori, especially those under the age of 65 years. The importance of the role of infection and inflammation in atherosclerosis is widely accepted, and there has been increasing awareness that immune responses are central to atherogenesis. Chronic inflammatory periodontal disease is among the most common infections, with over 10% of New Zealanders at risk of losing teeth. A number of studies have shown associations between periodontal diseases and an increased risk of stroke and coronary heart disease. While it is recognised that large-scale intervention studies are required, studies showing the pathogenic mechanism are still needed to establish the biological rationale underlying the relationship between these two chronic diseases.
Objective: The objective of the present study was to determine the profile of cytokine gene expression of P. gingivalis GroEL reactive T cells in the peripheral blood of patients with atherosclerosis and those with chronic periodontitis. In this way some insight may be gained into the functional activity of these cells in atherosclerosis and chronic periodontitis.
Methods: Peripheral blood was obtained from five atherosclerosis, five chronic periodontitis, and two healthy patients plus one patient with atherosclerosis with chronic periodontitis. Mononuclear cells were isolated using density gradient centrifugation. The mononuclear cells were cultured in media alone and with recombinant P. gingivalis GroEL to determine the lymphoproliferative activity in each group. RNA extraction was performed and followed by real time PCR to determine the cytokine gene expression profiles of atherosclerosis and chronic periodontitis patients. The results were analysed using the 2-ΔΔCt method.
Results: A variation of lymphoproliferative activity was observed in atherosclerosis and chronic periodontitis. Differential cytokine gene expression profiles were observed in the atherosclerosis and chronic periodontitis groups. Thirty-nine (46%) of 84 genes were differentially expressed in the atherosclerosis group, and fifty-five (65%) of 84 genes were differentially expressed in the chronic periodontitis group. Of the thirty-nine genes that were differentially expressed in the atherosclerosis group, nineteen (49%) were upregulated and twenty (51%) were downregulated. On the other hand, in the chronic periodontitis group, of the fifty-five genes that were differentially expressed, twenty-eight (51%) genes were upregulated, and twenty-seven (49%) genes were downregulated.
Conclusion: The results suggest that GroEL-specific T cells may respond differently in atherosclerosis and chronic periodontitis. The reason for this difference is unknown but may relate to the recognition of different epitopes in each group. This remains to be determined.

Introduction: The dental treatment in the Bolivian highlands is challenging. Due to the reduced infrastructure, the caries prevalence of the local population is heavily increased.
Objective: The aim of this pilot study was to investigate the general oral health situation of dental patients in a rural region of Bolivia.
Material and Method: 174 patients were included in this pilot study (age 24.64 ± 16.42 years, range 3 to 80 years; 65.36% female patients). Patients were treated in a dental office in Huancarani (operated by the FCSM). Patients lived in four different corporations. dmft-Index (0-6 years) and DMFT-Index, edentulousness situation, nutrition habits, and socio-economic factors were collected.
Results: The dmft-Index of the treated patients was 7.00. The DMFT-Index results in 8.02. 37.94% of the patients suffered from general dental plaque, and 60.34% showed isolated dental plaque. In 1.72% of the investigated patients, no dental plaque was detectable. 35.63% of the patients knew oral hygiene products. 4.02% of the study participants required treatment for an acute malocclusion. In 2.8% of the included patients, parasitoses (South American trypanosomiasis) were verified anamnestically.
Discussion and Conclusion: The oral health situation of the patients showed a massive restorative and prosthetic treatment demand. In addition, education on sufficient oral health and the dissemination of oral health products have to be pushed. The prevalence of South American trypanosomiasis, which can lead to death especially for children, can be assumed to comprise a considerable number of undetected cases. The main goal of further studies must be the development of interdisciplinary infection prophylaxis and oral health prophylaxis systems.

Die Fluoridexposition im Rahmen der täglichen Zahnpflege kann das galvanische Korrosionsverhalten von Implantatmaterialien empfindlich beeinflussen. Von besonderem Interesse ist dabei die Dynamik der Reaktion auf eine spontane Fluoridexposition.
Zur Untersuchung dieser Dynamik wurde die Reaktion der Kopplungen Ti/Ti und Ti/CoCrMo auf die Zugabe verschiedener Fluoridkonzentrationen sowohl im neutralen als auch im sauren Milieu mittels elektrochemischer Rauschmessungen aufgezeichnet. Anschließend erfolgten eine optische Oberflächenbewertung und eine Ermittlung des Massenverlustes an den Proben.
Während sich Ti/CoCrMo nicht beeinflussen ließ, zeigte Ti/Ti Veränderungen im galvanischen Korrosionsverhalten. Diese reichten - abhängig von pH-Wert und Fluoridkonzentration - von einer temporären Beeinträchtigung der Passivierungsschicht mit anschließender Repassivierung bis zur aktiven Korrosion mit signifikantem Massenverlust.
Aus den Untersuchungen folgt, dass bei Ti/Ti die Kombination von hoher Fluoridkonzentration und niedrigem pH-Wert zu vermeiden ist - insbesondere bei längerer Expositionsdauer. Dagegen ist Ti/CoCrMo gegenüber Fluoriden äußerst unempfindlich.

Introduction: Transmission of infectious diseases carries high risks for patients and dental professionals. Prosthetic work pieces have to be disinfected. To date, there are no specific disinfection products available for the one-shot disinfection process. Instead, universal disinfectants are used.
Objective: The aim of this study was to investigate the influence of chemical disinfectants in the one-shot disinfection process on material-mechanical parameters of several denture base resins.
Material and Method: 252 test specimens from four acrylic denture base resins (PalaXpress, Paladon 65, Futura Gen, FuturAcryl 2000) were produced. Test specimens were constructed prismatically according to DIN-EU-ISO 3167. Three chemical disinfecting agents were investigated (Impresept, D50 Instru-Gen, Stammopour DR). Specimens were stored in distilled water and divided into five test groups (2xcotrol, 3xtest). E-Modulus and flexural strength were detected using the three-point bending test.

Trotz moderner Therapieansätze ist das 5-Jahresüberleben von Patienten mit einem enoralen Plattenepithelkarzinomen (PECA) weiterhin konstant schlecht bei 50%. Die wichtigsten Prognosefaktoren sind hierbei die Tumorgröße und das Vorhandensein von Lymphknotenmetastasen (CM) zum Zeitpunkt der Erstdiagnose sowie die Entstehung eines Rezidivs im Verlauf der Nachsorge. Hinsichtlich des Rezidivverhaltens gibt es wenige Daten, wodurch eine frühzeitige Detektion in der Tumornachsorge extrem erschwert wird. Dies zeigt sich auch in einem geringen Evidenzlevel für die Vorgehensweise in der Tumornachsorge in der deutschen Leitlinie.
Retrospektiv über die Jahre 2000-2014 wurden die Patienten der Mund-, Kiefer- und Gesichtschirurgie der Universitätsmedizin Mainz, die an einem primären enoralen PECA erkrankten und in domo therapiert wurden, nachuntersucht. Neben der Untersuchung der epidemiologischen Daten, der Risikofaktoren und des TNM-Status lag das Hauptaugenmerk auf dem Rezidivverhalten und -typen sowie den Einflussfaktoren und der Diagnostik.
Insgesamt wurden 766 Patienten (34% Frauen und 66% Männer) bei einem Durchschnittsalter von 62±13 Jahren verglichen.
Bedingt durch die hohe Anzahl an T3-4 Karzinomen (33%) und der hohen Rate an Lymphknotenmetastasen (37%) hatten bereits 50% der Patienten ein fortgeschrittenes Tumorstadium von III-IV. Dabei entwickelten 70% innerhalb der ersten 2Jahre ein Rezidiv, mit einem Median von 12 Monaten innerhalb der untersuchten Personengruppe.
Der häufigste Rezidivtyp war das Lokalrezidiv mit 36%.
Über die klinische Untersuchungsmethode wurden (55%) der Rezidive diagnostiziert.
Das Vorliegen schlecht differenzierter Tumore und die Existenz von Lymphknotenmetastasen gingen statistisch signifikant häufiger mit der Entwicklung eines Rezidivs einher. Sowohl die fortgeschrittene Tumorausdehnung als auch das Vorhandensein von Lymphknotenmetastasen sowie das Rezidiv sind im Verlauf mit einem signifikant schlechteren 10-Jahresüberleben behaftet.
Somit kann man schlussfolgern, insbesondere bei Vorliegen von cervikalen Metastasen und undifferenzierten Primarien, dass das PECA der Mundhöhle häufig rezidiviert. Die vorliegenden Daten untermauern die absolute Notwendigkeit einer engmaschigen klinischen Nachsorge, insbesondere in den ersten 2 Jahren, die über den in den Leitlinien empfohlenen 3 Monatsrhythmus hinausgehen sollte, um Rezidive zeitnah zu detektieren.

Die Zahnbehandlungshobie kommt relativ häufig vor. Jüngere Studien haben gezeigt, dass 68% der Bevölkerung Angst vor dem Zahnarzt haben und 11% davon an einer Zahnbehandlungsphobie leiden. Im Gegensatz zur Zahnbehandlungsangst bedarf die Zahnbehandlungsphobie einer psychotherapeutischen Behandlung. Ziel der vorliegenden Studie war es den Erfolg von psychotherapeutischer Behandlung bei Patienten mit Zahnbehandlungsphobie zu erfassen.

During tooth development odontoblasts embed growth factors in the dentin matrix, where their potential for biochemical signaling is preserved. They can be liberated later by demineralising agents and might influence immune response as well as the behavior of pulp cells. The aim of this study was to investigate the impact of dentin matrix proteins (DMPs) on the migration, viability and mineralisation of human dental pulp cells.
At first, a complex procedure to isolate, purify and concentrate DMPs (up to 80,000 pg/ml TGF-β1) from human dentin was established. DMPs were utilised at different concentrations in a modified Boyden chamber assay to investigate chemotactic effects on pulp cells after 24 hours. Viability was analysed by MTT testing over a period of 14 days, and alizarin staining after 21 days provided information on the induction of mineralisation.
DMPs showed a concentration-dependent chemotactic effect on human pulp cells and were able to influence their metabolic activity. Furthermore, they induced mineralisation that indicated cellular differentiation.
These investigations show the potential of endogenous signaling molecules from human dentin to influence viability and enhance the migration as well as differentiation of pulp cells. The release of DMPs, which is easy to achieve in the clinic through the superficial demineralisation of dentin, might optimise procedures in vital pulp therapy as well as in regenerative endodontics in the near future.
