Eine Therapie mit festsitzenden funktionskieferorthopädischen Apparaturen ist bei ausgeprägter Klasse-II-Relation mit deutlicher skelettaler Ursache indiziert - dies gilt sowohl für die Klasse II,1 als auch für die Klasse II,2. Dabei kommt es zu einer Distalisierung des oberen und einer Mesialisierung des unteren Zahnbogens. Außerdem wird das Wachstum in Form einer Hemmung im Oberkiefer und einer Stimulierung im Unterkiefer mit entsprechenden Veränderungen im Bereich des Kiefergelenks beeinflusst. Die Behandlung mit festsitzenden funktionskieferorthopädischen Apparaturen hat sich als effektiv und zuverlässig erwiesen. Für die Herbst-Behandlung konnten auch eine sehr gute Langzeitstabilität (≥ 15 Jahre) und im Vergleich zur Normalbevölkerung positive Effekte hinsichtlich der Mundgesundheit ohne Auswirkungen auf die Kiefergelenkfunktion ermittelt werden. Aim: For orthodontic aligner treatment, excellent full-arch impressions with correctly displayed interdental areas (IAs) are required. To analyze the ability of impression taking of the IAs in periodontally compromised dentitions, two intraoral scanning systems and one conventional impression technique were investigated in vitro under standardized testing conditions. Materials and methods: A total of 60 impressions of the maxilla and mandible were taken from a periodontally compromised test model (A-PB) with three different techniques (n = 20): One conventional impression (EXA'lence) (CVI) and two digital impressions with the intraoral scanners Trios III (3Shape) (TIO) and True Definition (3M ESPE) (TRU). Standard tessellation language (STL) datasets were generated for TIO and TRU, whereas type IV dental stone casts were manufactured for CVI. The casts were then digitized with a laboratory scanner (ATOS). The percentage of displayed IAs in relation to the complete IA was calculated for each IA using evaluation software (GOM Inspect). Finally, the data were subjected to the median test. Result: TRU showed a significantly higher percentage of displayed IAs compared with the other two methods (P < 0.05). Only a few IAs were shown in CVI. TIO showed significantly better results compared with CVI, although the results were not as good as those of TRU. Conclusion: Within the limitations of this in vitro study, intraoral scanners - and especially the one based on active wavefront sampling (AWS) technology (as for TRU) - can be recommended for the reproduction of wide IAs (undercuts) in periodontally compromised patients. Background: This article is the result of a debate at the European Journal of Orthodontics Open Session in 2013 in Reykjavik, Iceland. Objective: The aim of this article is to highlight some of the strengths and weakness of clinical orthodontic research, with particular emphasis on randomized controlled trials (RCT). The ultimate aim of improving clinical orthodontic research in general. Design: This article is organized into two sections with arguments for and against RCTs. The backgrounds to evidence- based evaluation and the level or quality of evidence in trials are discussed. The article emphasises what makes high quality clinical research, and gives practical advice including examples of tips and potential pitfalls for those undertaking clinical research. Results and Conclusion: The overriding message is constructive and it is hoped that the article serves as an aid in evaluating, designing, conducting, and reporting clinical research. Vor allem in den Industrienationen steigt der Anteil der Kinder mit Übergewicht und Adipositas seit Jahren dramatisch an. Dass diese Kinder sowohl allgemeinmedizinisch als auch psychosozial eine erhöhte Aufmerksamkeit benötigen, ist mittlerweile unumstritten. Doch auch im Bereich der Zahnmedizin ist man auf besondere Risiken bei übergewichtigen Patienten aufmerksam geworden. So beschäftigten sich in den vergangenen Jahren diverse Studien mit dem Zusammenhang zwischen Übergewicht bei Kindern und Kariesbefall und auch im Bereich der Parodontologie ist Adipositas zunehmend in den Fokus gerückt. Sie wird mittlerweile als einer der Hauptrisikofaktoren für parodontale Erkrankungen gesehen. Im kieferorthopädischen Praxisalltag werden heutzutage wesentlich mehr übergewichtige/adipöse Kinder und Jugendliche behandelt als früher, doch die kieferorthopädische Forschung hat sich bislang nur wenig mit den Besonderheiten der Behandlung adipöser Patienten beschäftigt. Dabei sind die Berührungspunkte zur konservierenden Zahnheilkunde (White-Spot-Läsionen während Multibracketbehandlung) oder Parodontologie (Knochenresorption/-umbau) durchaus naheliegend.

Objectives: To assess the general dental health, orthodontic status, and orthodontic treatment need in young adults. Method and Materials: The subjects were 121 first-year dental students. Fifty-one individuals without orthodontic treatment were compared with 70 orthodontically treated individuals. It was differentiated whether orthodontic treatment had been performed by a certified orthodontic specialist or a general dental practitioner. DMF(T) and DM(T) Index values were recorded. Dental casts were analyzed, and the orthodontic treatment need was assessed using the Dental Health Component (DHC) of the Index of Orthodontic Treatment Need (IOTN). Results: Orthodontically treated subjects exhibited statistically significantly lower DMF(T) and DM(T) values than untreated individuals. Treatment performed by a certified orthodontist resulted in statistically significantly better treatment results with more Class I relationships and less crowding. The remaining degree of orthodontic treatment need was also lower in subjects treated by a certified orthodontist. A moderate to very high degree of treatment need (DHC degrees III to V) was present in 59% of the subjects treated by a general practitioner but only 29% of the subjects treated by a certified orthodontist. Conclusion: Orthodontic treatment improves general dental health. Treatment performed by a certified orthodontist leads to better treatment results and lower remaining treatment need compared with that performed by a general practitioner. Vom 20. bis 24. Juni 2007 tagte in der Berliner Philharmonie die European Orthodontic Society (EOS) gemeinsam mit der Deutschen Gesellschaft für Kieferorthopädie (DGKFO). Die EOS feierte dabei den 100. Jahrestag ihrer Gründung in Berlin, während die DGKFO anlässlich ihres 99-jährigen Bestehens ihre 80. Wissenschaftliche Jahrestagung durchführte. Der Einladung beider Gesellschaften folgten etwa 3.000 Kieferorthopäden aus 80 Ländern sowie mehr als 130 Aussteller, die auf 1.600 m2 in den Wandelhallen der Philharmonie ihre neuesten Produkte für Kieferorthopäden anboten. Neben dem wissenschaftlichen Programm fanden zahlreiche Treffen nationaler und internationaler Organisationen statt; zwei Vorkongresskurse und ein Nachkongresskurs im nahe gelegenen Maritim Hotel rundeten die Gemeinschaftstagung ab. Die Eröffnungsveranstaltung unterschied sich deutlich vom Üblichen, wurde sie doch mit aufwändiger Technik von professionellen Akteuren mit viel Musik und nur wenigen Reden gestaltet. Passend zum 100-jährigen Jubiläum lautete das erste Hauptthema der EOS: "What have we accomplished in orthodontics in the last 100 years?". Neben Kurzvorträgen wurden zu diesem Thema sieben Hauptreferate gehalten. Die meisten Referenten begleiten die Geschicke der Kieferorthopädie seit langem als Schriftleiter der großen internationalen kieferorthopädischen Fachzeitschriften. Das zweite EOS-Hauptthema lautete: "Failure, relapse and retention", während es im dritten Hauptthema um "IT and CAD/CAM" ging. Auf der DGKFO-Jahrestagung wurde eines der derzeit aktuellsten kieferorthopädischen Themen aufgegriffen - die skelettale Verankerung mittels Implantaten und Minipins. Nachfolgend soll über die Einführungsreferate zu den oben genannten Themen und die Sheldon Friel Memorial Lecture berichtet werden, die in diesem Jahr Prof. F. P. van der Linden hielt. Wer aber glaubt, es sei in Berlin nur gearbeitet worden, der irrt. Die President's Reception im Schauspielhaus, das Gala Dinner im Meilenwerk, umgeben von Maseratis und Ferraris, die Postgraduate Disco Night im Watergate sowie das Past Presidents Dinner auf dem Dach des Bundestages boten reichlich Gelegenheit zum Feiern.
